You’ll find forms on our website that you can download ahead of time. And, if you have a doctor referral or insurance for your treatment, make sure you bring the necessary paperwork with you.
During your first appointment, we’ll also complete an examination. This will give us the information we need to make or confirm a diagnosis, understand the current stage of your injury and get started on your personalized in-clinic sports recovery plan. As we develop your plan, we may also assign you home exercises to help you achieve your goals.
